Enter Medication

• Medication Name: Select or enter the medication name you want to create a reminder for.
  
• Dispense Date: Enter the date when this medication was dispensed/given to you by the pharmacy. If you don't have or know the information, click unknown.
• Duration: Select the duration in days/weeks/months. If it's for an unknown length of time, click unknown.
• Dosage: Select the dosage of the medication. Tablets/Puffs/Tsp/Tbsp/Shots
• Frequency: Enter the how frequently you will be taking this medication.
