In order to retrieve your medical information through a fax machine or for an emergency physician to lookup your information on the web, you will need the FAX ID along with a PIN number. The PIN is a number chosen by you when you registered as a Premier Member.

You will have the option to have the PIN printed on your card. If you choose not to put your PIN on the card and if you are unable to recall the PIN, you will not be able to retrieve your information in an emergency using a fax machine.

Keep in mind that it is often difficult for physicians to obtain this information if a patient is unconscious or in shock.
