Member Information required during this initial phase includes basic data; e.g.: name, address, date of birth and e-mail. This information will appear on the printout received by an authorized emergency physician. You must fill in all of the information in order to proceed to the next step.

Basic Information

During the registration process, you will be asked to provide basic data; e.g.: name, address, date of birth and e-mail. This information will become part of your secured PersonalMD file.

Emergency Contacts

During the registration process, you will be asked to provide the name and phone number for a family member or friend that a physician can contact in case of an emergency. You should choose someone who you believe would make appropriate decisions regarding your health in an emergency.
