up for Medication Reminder is as easy as 1, 2, 3!!
If you are
a PersonalMD.com premier member signing
up for medication reminder will take you only 5 minutes! STEP:1

Have your alphanumeric pager ready. We don't provide you with a pager. STEP:2
- Signup by filling out the Medication
Reminder Registration Form. Service is $14.95/year. STEP:3
- Your Medication Reminder Account is now active!
The
Registration Form requires you to fill out these particular fields:
1. Select Member
If you have a Family
Membership, you will need to select the name of the member you are signing
up for the Medication Reminder service. Scroll down the listing to find the member's
name.
This
service is per individual only.I.e: In a family account you cannot signup for
all members at once. 2.
Pager Email ID
Your pager email ID is the number given to you by your
pager service to identify your email account. If you don't have a pager email
ID, contact your pager service.
Remember, you will need an alphanumeric pager
(one that uses the alphabet as well as numbers).
If you have a Family
Membership, you will need to enter the Pager Email ID the member will be using;
it may be different from yours. 3.General
Preferences
Select
general preferences if indicated. Time
Zone: Please select your time zone. This is usually your home time
zone. If you travel from your home time zone, you will have to edit your time
zone preference to indicate your current time zone.
Daylight
Savings Time: For members who live in the United States, please select
your daylight savings time preference.
For most, if you select automatic,
we will automatically adjust times when Daylight Savings Time begins and ends.
However, if you live in state that does not change time, please select on or off
as appropriate.
4. Account Information
Enter the Credit Card information for the card
to which you will have this service billed. If you have any questions about the
billing information or terms and conditions, please read our billing
statement and terms & conditions for this service.
Card
Type: Select the credit card to which Medication Reminder services
will be billed. Name
on the Card:
Type in the name on the credit card as it appears on the front of the card. Card
Number: Type
in the credit card account number. Expiration
Date: Enter the expiration date of the credit card. Click
