Medical Information
Information requested during this phase includes primary physician's name and number, primary family contact's name and number, blood type, whether or not you're an organ donor, allergies and drug sensitivities.

This information will appear on the back of your PersonalMD-Emergency Card as well as on the printout received by an authorized emergency physician. Paramedics will be able to read this information en route to the hospital.

The main sections are:

Current Health : Information about your current health and medical status assists emergency physicians in determining possible causes and treatments for the problems which prompted your visit to the emergency department.

Allergies & Drug Sensitivities : Many people have adverse reactions to drugs commonly administered in the emergency department, including painkillers and medications for asthma and blood pressure. By providing emergency physicians with knowledge about your allergies and drug sensitivities, you are enabling them to choose appropriate treatments for your condition, thereby avoiding adverse and even life-threatening reactions.

Medications : Please provide a list of any and all medications your are currently taking. This will enable physicians to prescribe the most effective treatments for your current condition and help prevent adverse drug interactions.

Family History : Knowledge of your family's medical history provides physicians with additional information relative to your current medical problems. By providing this information in your PersonalMD file, you are equipping emergency physicians with important diagnostic tools.

Immunizations : Knowledge of your immunizations will help emergency physicians rule out possible health conditions, especially for children.

Medical Devices : Please list any medical devices you currently use, including pacemakers and dental appliances.
