can I ensure paramedics and emergency physicians can access my card in case of
an emergency? Be
sure to place your PersonalMD-ER Card in a prominent location in your wallet that
a paramedic or physician will see even before your health insurance card. In most
cases, you will be able to give the card to the paramedic or physician and they
will be able to access the information by following the instructions shown on

I have to give my doctor's staff my password to access my records or fax records
into my file?
No.
There is no reason to share your password with
the physician's office. Physicians
can fax the information into your file using only
your Fax ID and PIN code.

can I order additional cards?
You
can order additional cards by logging into your
account, clicking on the "Manage Account"
tab then just click on "Click here to order
Emergency Card."

is an Individual PersonalMD-Emergency Card? An
individual card contains all of the information you have provided for one person.
This card should be carried by the person whose name appears on the front. In
case of a child, it can be carried by an adult who is responsible for the child's
well being. Examples of this would be a daycare director, camp manager or school
authorities. We recommend that a parent carry the family card in addition to his
or her individual card. [Top] What
is a Family PersonalMD-Emergency Card?
A
family card contains the FAX ID and PIN numbers
for the whole family (up to ten people) on one
card. The front of the card will contain the names,
FAX ID and optionally PIN numbers (if desired).
Information on the back would include information
about the primary member's medical data. This
information is customizable as you desire.
