Medication
Name:
Enter the name of the medication. Use the name of
the medication that you are most familiar with.
You may use the generic (or chemical) name, the trade
(brand) name, or your description of the medication.
For example, if you are taking Lasix, you may enter
the medication name as furosomide (the generic name),
Lasix (the trade name), or water pill (what you call
it).
We recommend that you use the generic or trade name,
as in case of an emergency, the doctor may not know
the specific medication you are taking if you enter
water pill, as there are many types of water pills.
Dispense
Date: Select the Month, Day, and Year that
the medication was dispensed (given) to you from the
pharmacy. This information is usually located on the
prescription label. If you are entering a non-prescription
(over the counter) medication, enter the date when
you purchased that medication or click "Unknown".
Duration:
Select the duration (length of time) that you will
be taking the medication. This information is usually
found on the prescription label or told to you by
your doctor.
For example, if the label says, "Take for 10 days",
then enter 10 days.
If don't know the duration or are taking it for a
undefined length of time, click "Unknown".
Dosage:
Enter the dosage as written on the prescription
label or in the case of a non-prescription medication,
as directed by your doctor, for example, 500 mg or
2 tablets.
We recommend that when possible, you use the actual
dosage (500 mg, 2 ml, etc) rather than the number
of tablets or capsules, as in case of an emergency,
the doctor will better know the specific dosage of
the medication.
Frequency:
Enter the frequency with which you will take the medication.
For example, 4 times a day, or once a week.
