How can PersonalMD Emergency Card help you, help others?
   

How can I use the PersonalMD-ER Card to give patients better care en route to the hospital?

 

There are two ways the PersonalMD ER Card helps you care for the patient.

  1. You will find information on the back of the card that will be of immediate assistance to you, including the patient's blood type, allergies and drug sensitivities. This will guide you regarding which medications can be administered en route.

  2. The front of the card contains the patient's name, Fax ID and PIN Code. You should let the emergency department personnel know this information while en route so they can access vital medical data, including EKGs and x-ray reports, before you arrive at the hospital. The back of the card also contains the names and numbers for the patient's primary physician and primary family contact, which should be relayed to the hospital in dire emergencies.
   
How does the hospital access the information from the patient's PersonalMD.com file?
 

Once the patient gives you permission to access his information, and you obtain the patient's FAX ID and PIN numbers, there are two ways to access the medical information from the patient's PersonalMD.com

1. Using the Internet

2. Using PersonalMD's Automated Fax Back System

Using the Internet:

Go to www.PersonalMD.Com and log in using the FAX ID number or User Name and PIN (for password). This will give you access to the member's medical information. Please note that this is READ-ONLY access and you will not be able to change or edit any information in the record.

Instructions for using the automated fax back system:

1. Go to a fax machine and pick up the hand set or press the speaker phone button (some fax machines have a "Hook" button or "Phone" button).

2. Once you hear the dial tone, dial 1-925-460-9090. You will hear a welcome message -- press 1 to confirm that you are calling from a fax machine.

3. At the prompt enter your Fax ID and PIN number. After your FAX ID and PIN have been confirmed and verified, the system responds with "Press 1 to retrieve your medical information" or "Press 2 to send a document".

4. Press "1" and the system will respond asking you to press the START button on your fax machine. After the system responds with "Press the Start button", you may hear a beep. Press the "SEND" or "START" button located on your fax machine and the documents will be faxed to you.

 
   
Can I provide the information to the emergency department personnel if the patient is unconscious or otherwise unable to grant authorization?
  Yes, if the local laws permit it. Although we would like the members to authorize permission to use this information, there are times when that is not humanly possible. Members understand and appreciate the fact that the card makes it possible for you and the physicians to give them the most efficient and effective care humanly possible if they are unable to respond to your questions and the time saved by the card may be a difference of life and death.
